ADMISSION TO THE HOME
Prospective Residents must meet the assessment criteria for high-level residential care. This assessment is carried out by an appointed Aged Care Assessment Team. Your local doctor or Social Worker at your nearest hospital can refer a prospective Resident to the appropriate Aged Care Assessment Team.
This health team will assess the prospective Resident and complete the NH5 form and decide whether the Resident is eligible for high level nursing care and admission to the Home. The form when completed is sent to the Department of Health, Housing, Local Government and Community Services for processing and final approval.
Prior to admission to the Home, the Clinical Services Coordinator will interview prospective residents and their families to complete an application for admission form. Admission to the Home can only be undertaken with a current NH5.
